Emergency Information Card - Keeping Information Current

8/1/2013

Please download and complete an electronic copy of the emergency card send it to school with your child during the first week.

Emergency Information Card English and Emergency Information Card Spanish Information Cards Available for download in Download Documents - Click Here.

1. Download the form.
2. Complete the form.
3. Print the form.
4. Send the form to school with your child.

*Using a computer to complete the emergency card helps ensure that the information is clear and legible for staff to read during an emergency.

Every child must have a emergency card on file at school. It must have up to date phone numbers with people who can come and pick up your child if you are unavailable. It is important that you update any phone numbers or address changes. This is especially important when it comes to who we can release your child to in the case of an emergency such as earthquake, etc. If you have a change of address, please submit a new proof of address (such as DWP or gas bill).
